A big part of getting care you need is making it to your appointments. If you’re a Medallion Medicaid or Anthem HealthKeepers Plus Commonwealth Coordinated Care Plus (Anthem CCC Plus) member, you get no-cost transportation to your doctor visits and other medical appointments. FAMIS members can also get professional ambulance services with prior approval.

For Medallion Medicaid and CCC Plus members,* we offer unlimited rides to doctor’s appointments and the pharmacy to pick up medication.

*Nonemergency rides are NOT a benefit for FAMIS members at this time. If you need emergency transportation, please call 911.

If you’re an ID or DD waiver member in CCC Plus, you’ll continue to use Logisticare through the Department of Medical Assistance Services (DMAS) fee-for-service for all waiver-related needs. We’ll cover Southeastrans transportation for acute services like hospital visits, physician appointments, and rides to the pharmacy. Contact your care coordinator for more details.

As an extra benefit, we also offer nonmedical rides for CCC Plus members.

We’ll get you to health and wellness activities like:

  • Places of worship
  • Grocery stores
  • Libraries
  • The DMV

Nonmedical rides are available seven days a week from 8 a.m. to 5 p.m. You qualify for 12 round trips* annually or three every three months. Just follow the steps above to set up your ride.

*A round trip is classified as one pickup and one drop-off not exceeding more than 30 miles round trip for urban locations and 60 miles for rural areas.

How to set up your ride

We contract with Southeastrans to help you get where you need to go. To set up your ride, just follow these steps:

1. Call Southeastrans five business days in advance of the requested date for medical appointment transportation.

Or schedule your ride online.

Please have your member ID number, address and date and time of your appointment ready when you call.

Remember to write down your confirmation number in case you need to refer to it later.

2. Let them know if you have any special transportation needs.

3. Go ahead and schedule a return trip if you know when your appointment will be over.

They’ll pick you up within 30 minutes. If you don’t know how long you’ll be, just call Southeastrans when your appointment is over.

Having issues with your ride?

If you can’t coordinate a ride or have a complaint, call out Member Services team at 1-800-901-0020 (TTY 711) if you’re Medallion Medicaid or FAMIS member, or at 1-855-323-4687 (TTY 711) if you’re a CCC Plus member.

Mileage Reimbursement Process

Southeastrans will reimburse you or your caregiver if a personal car is used. To receive mileage reimbursement, call Southeastrans five business days in advance of your appointment. You’ll receive the reimbursement form by mail or fax, and your provider must sign the form to verify the appointment.

For recurring transportation needs

If you need recurring rides to critical dialysis services, substance abuse treatment and adult day care, your providers need to contact Southeastrans and have them complete a form that will be approved for three-months at a time.
