Medallion Medicaid

Who may qualify?

  • Low-income women who are pregnant and their children
  • Children in foster care or adoption assistance
  • Uninsured adults and families who meet income guidelines

What are the income guidelines?

Dependent on household income and family size.  Check current income guidelines.

Children in foster care are automatically eligible (no income requirements).

What are the benefits?

  • Medical benefits like doctor visits, emergency and urgent care
  • Behavioral health services
  • Prescription and over-the-counter drugs
  • Vision care
  • Rides to appointments and the drugstore

FAMIS

Who may qualify?

Children under age 19 in families who may not qualify for Medicaid, but meet FAMIS income guidelines

What are the income guidelines?

Guidelines depend on household income and family size.  Check current FAMIS income guidelines.

  • Well-baby and well-child checkups
  • Behavioral health services
  • Prescription and over-the-counter drugs
  • Vision benefits

CCC Plus

Who can get it?

People with Medicaid and:

  • Medicare, too.
  • Live in a nursing facility.
  • Get long-term services and supports.
  • Are aged, blind or disabled.
  • Get care through any of the Developmental Disability waivers.

What are the income guidelines?

If you meet Medicaid income guidelines and are in one of the groups above, the state will automatically sign you up in CCC Plus.

  • A full range of medical benefits, like urgent and emergency care
  • Long-term services and supports
  • Your own care coordinator and care plan
  • Prescription and over-the-counter drugs
  • Vision and dental care
  • Rides to appointments and the drugstore
