Getting Started

We’re glad you’re an Anthem HealthKeepers Plus member. Here’s what to expect after you sign up.

Look for your Anthem HealthKeepers Plus member ID card in the mail.

Keep the card with you — you’ll use it to get services like doctor visits, prescriptions, getting care in your home or emergency care. You can print a temporary card, request a replacement, or download our mobile app to have your ID card ready on your phone whenever you need it.

Your doctors and care team

Your primary care provider (PCP) is listed on your ID card. This is your main doctor who will coordinate all your medical needs. He or she will be your first contact when you need care.

If you’re a CCC Plus member, your care coordinator is there to work with your PCP and other doctors to manage your care. Reach out to your care coordinator if you need help with long-term services and supports or if your needs ever change.

Help us learn about you

If you’re a Medallion Medicaid or FAMIS member, you should have gotten a Health Assessment (HA) in your new member packet. The HA tells us more about your health, so we can help connect you with care and doctors that fit your needs. Our Health Assessment (HA) is a short questionnaire. It won’t take more than five minutes to fill out.

Fill out the health assessment

Need help filling out the HA? Call 1-800-901-0020 (TTY 711).

If you’re a CCC Plus member, your care coordinator will help you fill out your HA.

Reporting your changes

Let us know if there’s a change in your status, so we can tell the state your new information. If you have a baby, move to a new address or change jobs, call the Member Services number on your member ID card or update your information on your secure account.
