Your Individualized Safety Plan

Our Safety Engagement Program (SEP) is designed to help you achieve your safety goals. Anthem
Blue Cross and Blue Shield Healthcare Solutions has partnered with Youth Advocate Programs, Inc. (YAP) to offer you support, improve your safety and reduce the risk of violence-related incidents in your life.

We do this by:

• Working closely with you to develop specific goals
• Coordinating services with community organizations and support systems, like schools, social services organizations and more

Safety Engagement Coordinator

You’ll be assigned a Safety Engagement Coordinator (SEC) who will visit with you to introduce the program. He or she will talk with you to learn more about your and your family’s needs, strengths, goals and priorities. We’ll first work with you on a plan for your immediate safety issues. Then, we’ll work with you to create an Individualized Safety Plan based on your strengths and needs. This plan will have clear goals and action steps for addressing your safety concerns.

Your SEC will also track your progress toward completing your Individualized Safety Plan goals and actively prepare you for graduating from the program.
