Talk with your doctor about preventive care that’s right for you.

You can earn incentives through our Healthy Rewards program when you complete important actions to help you stay at your best.

• Take the Health Needs Screening
• Complete recommended preventive visits (including dental) and screenings for your age as recommended by your doctor
• Receive recommended vaccinations

• Get an annual flu shot
• Take your child for a lead screening before age 2
• Engage in Care Coordination services

• Connect with your provider as soon as you know you are pregnant. Prenatal and postpartum care are critical for you and your baby.
• Quit smoking or use of other tobacco products
• Manage your chronic conditions

Healthy Rewards Program Rules

• Medicaid must be your primary insurance.
• You must be an eligible Anthem member at the time the reward is used.
• Your Healthy Rewards may only be used at participating retailers in Indiana.

• The purchase of alcohol, tobacco, e-cigarettes, firearms, or prescription drugs is not allowed.
