Health Needs Screening

Earn up to $30 in Healthy Rewards.

Complete the health needs screening (HNS) within 30 days of becoming an Anthem member, and we’ll give you $30 in Healthy Rewards (or $20 for completion between days 31 and 90). To earn your rewards, you must complete the HNS within 90 days of joining Anthem.

Grievances and Appeals

We care about the quality of care you get from your doctors and us. If you’re not happy with the care you get from one of the doctors in your plan, please let us know. There are two ways you, or someone you choose to act for you, can let us know your problem:

Our Member Services staff will try to take care of your problem right away.

If you want to file an appeal about how we solved your problem, it can be requested within 60 calendar days from the day of our decision on the grievance resolution letter.

Right Choices Program

If you’re eligible for the Right Choices program, you’ll have access to providers who will help you manage your health care needs. This team of experts will help you use your services the right way, so you feel better. The RCP lasts for two years. Within that time, a nurse care manager will help you:

  • Learn how to best use your medical benefits.
  • Get the best-coordinated care.
  • Have a medical home that will help keep you healthy and safe.
