Long-Term Services and Supports (LTSS)
Don’t just feel well. Live well.
Need help with grocery shopping and making meals? Getting cleaned up and dressed? Going to your doctor appointments? As part of Long-Term Services and Supports (LTSS), we help you get the home- and community-based services to help you stay in your home, with your family and friends.
Community-Based Adult Services (CBAS)*
You may qualify for CBAS if you:
- Are 18 years or older
- Have health problems that make it hard for you to take care of yourself and you need extra help
If you qualify, we’ll send you to the CBAS center that best meets your needs. If there is no center in your area, we’ll make sure you get the services you need from other providers.
Services may include:
- Skilled nursing care
- Social services
- Meals
- Therapies
- Family/caregiver training and support
- Rides
In-Home Supportive Services (IHSS)*
IHSS allows eligible seniors and persons with disabilities to hire a homecare worker to assist them with their activities of daily living, instrumental activities of daily living, and other personal needs so they can remain safely in their homes.
You may qualify for IHSS if you:
- Are age 65 years or older or are blind.
- Have a disability.
- Are unable to live at home without help.
IHSS allows you to hire a caregiver to help you with your daily needs so you can remain safely in your own home.
Services may include:
- Meal prep and cleanup
- Laundry
- Personal care services (such as bathroom assistance, bathing, grooming, and paramedical services)
- Grocery shopping and errands
- Rides to medical appointments
- Household and yard cleaning
- Accompaniment to medical appointments
- Protective supervision
If you currently get IHSS, you do not have to change your IHSS providers, and you can still hire, fire, and manage your providers. There will be no change in how your provider is paid.
* Certain eligibility requirements must be met to qualify for these programs. IHSS is a statewide program run by each county and the California Department of Social Services.
Multi-Purpose Senior Service Program (MSSP)*
Multipurpose Senior Services Program (MSSP) is provided in your home and helps arrange assistance and managing services, including adult day care, home modifications, and care management services. Medical care is not covered under this waiver.
You may qualify for MSSP services if you:
- Are 65 years or older.
- Have a disability.
- Are eligible for nursing facility placement but wish to remain at home.
Services may include:
- Adult day care
- Help paying for housing
- Chore and personal care help
- Protective supervision
- Caregiver respite
- Care management
- Rides to medical appointments
- Home delivered meal services
- Social services
- Communication services
If you currently get MSSP, you do not have to change your MSSP provider.
MSSP is a waiver program run by the state and available through a provider in your geographical area.
* Certain eligibility requirements must be met to qualify for these programs.
Long-Term Care (LTC)*
If you need more care than these programs can provide, your service coordinator can work with you to find other LTC options in a skilled nursing facility or similar location. To receive LTC services, a written request must be made by your primary care provider (PCP).
If you are currently in a skilled nursing facility, you do not have to change your facility.
* Certain eligibility requirements must be met to qualify for these services.
