Amador, Los Angeles, Madera, Mariposa, Mono, Nevada, Sierra and Tulare Counties (Medi-Cal and MRMIP)

Kern, San Bernardino, San Luis Obispo, Santa Barbara and Sonoma Counties (MRMIP only)
