Health Needs Screening
Earn $10 in Walmart purchases on your Anthem Rewards Card! Complete the Health Needs Screening (HNS), and Anthem Blue Cross and Blue Shield will give you $10 on your Anthem Rewards Card! To earn your rewards, you must complete the HNS within 90 days of joining Anthem.

Our Member Services staff will try to take care of your problem right away.
If you want to file an appeal about how we solved your problem, it can be requested within 33 calendar days from the day of our decision on the grievance resolution letter.
