Health Needs Screening

Earn up to $30 in Healthy Rewards.

Complete the health needs screening (HNS) within 30 days of becoming an Anthem member, and we’ll give you $30 in Healthy Rewards (or $20 for completion between days 31 and 90). To earn your rewards, you must complete the HNS within 90 days of joining Anthem.

New To Anthem?

As a new member, we know you have questions. We also know you’re short on time! Learn the basics about your health plan on the New Member Welcome page.

We care about the quality of care you get from your doctors and us. If you’re not happy with the care you get from one of the doctors in your plan, you or someone you choose to act for you can let us know your problem.

Filing a Grievance

If you have a complaint or problem with the care you are getting, you can file a grievance with Anthem. A grievance can be filed in writing, or by phone. A grievance can be filed with Anthem at any time.

Filing an Appeal

If you want to file an appeal about how we solved your problem, you must do so within 60 calendar days from the day of our decision on the grievance resolution letter.

The RCP lasts for two years. Within that time, a nurse care manager will help you:

  • Learn how to best use your medical benefits.
  • Get the best-coordinated care.
